I am aware of the possibility of these injuries and in joining a training session (or allowing my child to participate in a training session). I take full responsibility for this decision, further I am cognisant of the consequences of this decision. • I hereby (on my behalf and/or on behalf of the child) release and discharge PDK Dojo Karate, its Instructors, officers, staff and administrators to the maximum extent permitted by law from; a) All responsibility or liability from any act, omission, or neglect causing loss or damage to (myself) and/or (child) personal property or injury to (myself) and/or (child) or death to (myself) and/or (child). b) All claims, suits, actions or proceedings or demands, costs, charges and liabilities now or in the future of whatsoever nature, arising out of or in any manner whatsoever connected to (myself) and/or (child) with participation in PDK Dojo Karate activities from the date to (myself) and/or (child) first participates in PDK Dojo Karate activities/training session onwards. • I unconditionally and irrevocably agree to indemnify PDK Dojo Karate for any loss, liability, cost, charge and expense, claim, suit, action or proceedings or demand now or in the future of whatsoever nature suffered or incurred by PDK Dojo arising out of or in any manner whatsoever connected with child’s participation in the Dojo activities.
